What is DEVA?

DEVA stands for Dependent Eligibility Verification Audit.

The San Francisco Health Service System (SFHSS) conducts DEVA audits to help maintain costs for health care benefits. Covering ineligible dependents puts health plan administrators at financial risk and increases overall healthcare costs for eligible members. Section E of the SFHSS Rules states that SFHSS may require proof of dependent eligibility at any time.

During a DEVA audit, a portion of the members will be asked to provide documentation to verify their relationship with their dependent spouse, registered domestic partner, or children.

Failure to provide proof of dependent eligibility within the specified deadlines will result in the audited dependent(s) being terminated from health plan coverage. Note that coverage terminated for a domestic partner will terminate coverage for any dependent children of the domestic partner. Coverage that is terminated for a spouse will terminate coverage for the spouse’s children, who are your stepchild(ren).

Three Steps to Verify Your Dependent’s Eligibility

Step 1

Review the List of Acceptable DEVA Documents for your eligible dependent(s).

Step 2

Gather all necessary documentation that’s needed to complete your audit. Redact any irrelevant information.

Step 3

Submit all the acceptable documents. Log on to your eBenefits or do the other options: Fax, Mail, Drop-Off.

  • Submit by eBenefits

    Log into the SF Employee Portal at sfgov.org/sfc/employee-gateway and select the My Links. Under the Employee Links section, you will see the eBenefits section and under that, click on Submit a Qualifying Life Event.

    From there, click I Received a DEVA Notice.

    Follow the instructions to upload your verification documents or remove your ineligible dependent(s).

    ATTENTION: WHEN UPLOADING YOUR DOCUMENTATION, YOU MUST CONTINUE THROUGH ALL THE SCREENS AND CLICK ON THE SUBMIT BUTTON BEFORE EXITING. OTHERWISE, YOUR UPLOAD WILL NOT BE COMPLETED.

  • Submit by Fax, Mail, or Drop-Off

    You can fax your documentation with the enclosed cover sheet to (628) 652-4701.

    Mail or drop-off a photocopy of your documentation, along with the cover sheet, to:

             SFHSS – DEVA
             1145 Market St., 3rd Floor
             San Francisco, CA 94103

    The drop-box is accessible Monday through Friday from 9 a.m. to 4:30 p.m. and is located on the right side of the elevator bank on the 3rd floor.

    The cover sheet is only necessary if you choose the option of submitting your documents via fax, mail, or drop-off.
