HEALTH NET CANOPYCARE HMO WILL EXIT CA MARKET IN FEBRUARY 2027
A narrow network plan that provides care through a small number of local accountable care organizations (ACOs), a network of doctors and hospitals that share responsibility for providing care to you and your covered dependents. Under the Health Net CanopyCare HMO plan, you must receive all of your care from the Health Net CanopyCare HMO network of contracted medical groups and hospitals. You select a primary care physician within the Health Net CanopyCare network.
Refer to the Evidence of Coverage for a detailed list of services, exclusions and limitations. You must live or work in one of the zip code service areas served by the Health Net CanopyCare HMO plan.

The RealAge Program is a healthy behavior program targeting four key lifestyle risks: stress, sleep, nutrition, and activity. The program is personalized based on each individual’s risk level in each lifestyle category, as determined through RealAge test responses and personal interests. It is fully integrated with other features of the Sharecare platform, such as trackers, to encourage sustained engagement and promote behavior change that can lead to a lower RealAge.
Wellness Reward
Eligible members age 18 and older can earn annual gift card rewards of up to $100 by completing the RealAge Test ($50), sharing data with their PCP or completing three health coaching calls ($25), and completing two online Sharecare offerings ($25).
